Special Children's Friends, Inc.


Case management is part of community support for children and youth with special needs and their families. A case manager will listen to family's concerns and offer resources and support. Case management services include:

supporting and assisting famlies in their hard work to access services for their children
attending meetings with families
working with families to identify strengths and needs
coordinating treatment plans
monitoring services to be provided
advocating for services in the plan

Who is eligible?
Children, birth-20, who meet the definition of mental retardation, developmental delay or pervasive developmental disorder are eligible for SCF case management services. SCF will enroll children/youth or help families find alternate case management services and resources. Once enrolled, and as needed, SCF case management can continue until a young person reaches 20 years of age.

 

There is no cost to families with MaineCare. Others may have access to a sliding fee scale.

 

SCF provides case management services through a contract with the Department of Health and Human Services.
